Stronger For Life: The Active Sussex Reconditioning Project

The Active Sussex Stroke Reconditioning Project was established in response to the urgent need to address hospital-associated deconditioning, a major barrier to recovery for stroke survivors and older adults with complex needs.

Stroke continues to be one of the leading causes of long-term disability in England, and prolonged inactivity during hospital stays can significantly impair patients’ physical, emotional, and cognitive rehabilitation.

Recognising this challenge, East Sussex Healthcare NHS Trust, in collaboration with Active Sussex, Rother District Council (Active Rother), and East Sussex Public Health, developed a six-month pilot to test an integrated, community-based model of reconditioning care for patients at the Irvine Rehabilitation Unit.
