The event will provide the opportunity for healthcare professionals and instructors working with people with long term health conditions, to connect and build meaningful relationships that will help to improve local pathways and opportunities for people with long-term health conditions to move more.

Join us to expand your professional network, exchange best practices and challenges, and to discuss how to work more collaboratively to achieve greater health outcomes. We also want to get your thoughts on the future of the network and highlight specific development and support needs.
